一、為什麼要從「腳」開始看1. Why start at the feet

很多人腰痠、肩頸緊,第一反應是去處理腰和脖子。但其實最底下的地基——腳和椅子——若沒擺好,上面的每一節都得歪著補。就像照相機腳架有一隻腳短一截,上面的雲台怎麼調都不會平。所以這份清單,從地板往上排。When the back aches or the neck tightens, the first instinct is to fix the back and neck. But the foundation — feet and chair — if not set right, every joint above leans to compensate. Like a tripod with one short leg: the head can't be levelled no matter how you turn it. So this checklist starts at the floor and works up.

二、腳與椅:地基兩件事2. Feet and chair: the two foundation items

  • 腳要踩得到地。椅子太高、腳懸空,骨盆會被往前拉、腰容易蜷;踩不到就加個腳凳,讓大腿水平、腳掌貼地。Feet on the floor. A seat too high leaves the legs dangling, dragging the pelvis forward and curling the back; add a footrest so the thighs sit level and the soles plant.
  • 鞋子要穩。辦公室裡穿容易滑掉的拖鞋或過軟平底,腳會不自覺內塌,把歪斜一路傳到膝和腰。一雙平穩包覆的鞋,比任何護腰都直接。Shoes that grip. Slide-off slippers or flimsy flats at the office let the foot roll inward, sending the tilt up through knee and back. A stable, snug shoe helps more directly than any back support.
  • 椅面高度:膝髖同高。這一點在前面「開車族」也提過,辦公室一樣適用。膝蓋高過髖,腰就會蜷;太低則屁股滑前。Seat height: knees level with hips. Raised earlier for drivers, it applies at the desk too. Knees above hips curls the back; too low and the butt slides forward.
  • 腰背貼合椅背。腰後放小靠墊或捲毛巾,維持自然弧度,別整個人往後癱、讓腰懸空。Back against the backrest. A small cushion or rolled towel behind the waist holds the natural curve; don't collapse backward and leave the lower back hanging.

三、螢幕與鍵盤:手和頭的位置3. Screen and keyboard: where the hands and head go

  • 螢幕上緣約在眼睛高度。太低頭會往前探,太多「頭前傾」的拉力就從這裡來(詳見〈緊繃型頭痛與脖子〉)。筆電務必墊高或接螢幕。Screen top near eye level. Too low and the head drifts forward, sourcing much of that "forward-head" pull (see the tension-headache piece). Prop up the laptop or use a monitor.
  • 鍵盤滑鼠高度,讓手肘約成 90 度。手肘懸空或聳肩去夠,肩頸交界會一直緊。滑鼠別放太遠,手不用時離開滑鼠、回到扶手或桌面支撐。Keyboard and mouse at elbow-height, about 90°. Hovering elbows or reaching up with the shoulders keeps the neck-shoulder junction tight. Don't push the mouse far; when the hand rests, lift it off the mouse onto a support.
  • 腕部不要一直壓在桌緣。手腕長時間卡在硬桌邊受壓,是手部不適常見來源;用軟一點的護腕墊或調整高度讓手腕平順。Don't rest the wrists on a hard desk edge. The wrist pinned against a hard rim for hours is a common source of hand discomfort; use a softer rest or adjust height so the wrist runs level.

四、上半身:肩、胸、頭4. Upper body: shoulders, chest, head

  • 肩膀放鬆、不聳不扣。緊張或專注時,肩膀容易縮起來、往前扣。每隔一陣子刻意把肩膀往下沉、往後打開。Shoulders down, not hunched or rounded. Under stress or focus the shoulders creep up and forward. Every so often, deliberately drop and open them.
  • 胸口別整個塌進去。含胸會讓上背圓起、頭更往前。想像胸口微微往上、往前撐開,但不挺腰。Don't let the chest collapse. Rounded upper back pushes the head further forward. Imagine the chest gently lifted and opened — without arching the back.
  • 頭落在肩膀正上方。耳朵約和肩峰同一條垂直线,而不是跑到身體前面。這一項做好了,後頸的「加班」會少一大半。Head above the shoulders. The ear roughly over the shoulder tip, not in front of the body. Get this right and the neck's "overtime" drops by more than half.

一個 30 秒自我檢查A 30-second self-check

坐著的時候,從側面想像一條鉛垂線:耳垂—肩峰—髖骨大轉子—膝—踝,最好大約落在同一條線上。哪一段明顯往前或往後凸,哪一段就是今天要先顧的。Seated, picture a plumb line from the side: earlobe — shoulder tip — hip — knee — ankle should fall roughly on one line. Whichever segment juts clearly forward or back is the one to mind today.

五、比「擺好姿勢」更重要的一件事5. The thing more important than "perfect posture"

沒有「永遠正確」的姿勢,只有「維持太久」的姿勢。再標準的坐姿,坐滿八小時也會累。真正關鍵的是變換:定時起身、換個重心、做幾下伸展。研究與經驗都指向同一件事——頻繁地從同一個姿勢裡跳出來,比追求一個靜止的「完美姿勢」有用得多。There is no "always correct" posture, only a posture "held too long." Even the ideal seated position tires after eight hours. What really matters is changing: stand on a timer, shift weight, stretch a little. Research and experience say the same — breaking a fixed posture often beats chasing one perfect static pose.

  • 每小時起身一次。去倒水、拿印表機、原地踏步都好;重點是讓受壓的組織恢復循環。Stand once an hour. Get water, reach the printer, march in place — the point is to let compressed tissue recover its circulation.
  • 打電話時站起來走。通話那幾分鐘,是最自然的活動空檔。Walk while on the phone. Those call minutes are the most natural movement break there is.
  • 午後做一組全身伸展。從腳踝、腿後側、髖、肩頸一路拉一遍,比單點處理更見效。One full-body stretch in the afternoon. Ankle to hamstring to hip to shoulder-neck beats working a single point.

六、您自己可以先觀察的兩件事6. Two things to notice yourself

  • 哪一段最先不舒服?腳踝?腰?肩?哪一節先叫,就先從那一節的地基查起——多半不是它自己出的問題。Which segment complains first? Ankle? Back? Shoulder? Whichever speaks first, check the foundation beneath it — the trouble usually isn't there alone.
  • 休息後有沒有比較好?會隨起身活動明顯起伏的緊繃,多半是姿勢與久壓;完全不理會活動的,就要提高警覺、考慮就醫。Does rest help? Tightness that clearly eases with movement is mostly posture and pressure; tightness that ignores movement deserves attention and a medical look.
