「滑鼠手」和之前談過的肩頸僵硬,是同一類問題——最重要的,都出在桌面與座椅高度的配合。"Mouse hand" is the same family of problem as the neck and shoulder stiffness we wrote about — in both, what matters most is how the desk and chair meet.

一、所謂「滑鼠手」,其實是上肢力線的問題1. "Mouse hand" is really about the upper-limb line

大家口中的「滑鼠手」,指的通常是手腕、手指在長時間操作之後,出現的痠、脹、麻、使不上力,或動起來卡卡的感覺。它很少是「手腕自己壞了」,多半是整條手臂的力線沒順——從肩、肘、腕到手指,哪一處一直被逼著出力,久了就會在末端表現出來。What people call "mouse hand" usually means the sore, tight, tingling, weak or catching feeling in the wrist and fingers after long use. It's rarely "the wrist breaking down on its own" — more often the whole arm's line is off. From shoulder to elbow to wrist to fingers, wherever is forced to work overtime shows up at the far end.

所以與其盯著手腕那一点,不如先看:打字的時候,手肘在哪裡、前臂有沒有依託、手腕是不是一直過度彎折。這正是下面三個提醒的起點。So instead of staring at the wrist, look at this: where is the elbow when you type, does the forearm have support, and is the wrist bent too far for too long. That is exactly where the three reminders below begin.

先做分類,再做處理Sort first, then act

手腕不舒服的時候,第一件該做的事是先分清楚「這一種」和「那一種」,而不是先想怎麼把它弄鬆。這一點,比後面三個提醒都重要。When the wrist is uncomfortable, the first move is to tell "this kind" from "that kind" — not to reach for a way to loosen it. This matters more than the three reminders that follow.

這一類,是我們平常在處理的:This is the kind we work with every day:

  • 長時間用滑鼠、打字之後才出現,起來動一動、調整桌椅就鬆些Shows up after long hours on the mouse or keyboard, and eases once you move or fix the setup
  • 忙的時候明顯、放假休息就好些Worse when busy, better after a break or holiday
  • 只在某個角度或姿勢會卡,避開就沒事Only catches at one angle or posture; fine if you avoid it
  • 單純的痠、脹、緊,沒有其他的伴隨狀況Just sore, tight or swollen-feeling, with nothing else going on

這一類,請先讓醫師檢查:This is the kind to check with a physician first:

  • 手或手指麻、無力「真的做不到」——手抬不起、東西會掉,而不是只有感覺怪Hand or finger numbness or weakness you genuinely can't work around — an arm that won't lift, objects dropping — not just a weird feeling
  • 麻或無力的範圍持續加重逐日擴大Numbness or weakness that is steadily worsening or spreading day by day
  • 在車禍、跌倒等外傷之後才開始,而且一直沒好轉Began after a collision or fall and simply hasn't improved
  • 伴隨頸肩往手臂放射、且越來越明顯,或同時有發燒、體重不明下降Comes with pain radiating from neck and shoulder down the arm and growing, or with fever or unexplained weight loss

先做分類,再做處理——順序反過來,才是真正危險的地方。這兩組並不固定:同一個人也可能從左邊走到右邊,所以每次來訪,我們都會重新看一次。Sort first, then act — reverse that order and you're in the dangerous part. The two groups aren't fixed: the same person can move from one to the other, so we look again at every visit.

二、三個日常小提醒2. Three everyday reminders

絕大多數的手腕不舒服,都和「怎麼坐、怎麼用」有關。下面三件事,不需要買什麼特別的東西,今天就能開始。Most wrist discomfort comes down to how you sit and how you use your hands. None of the three below needs special gear — you can start today.

  1. 桌面與座椅高度的配合,是最重要的一件事。理想狀態是:用鍵盤或滑鼠時,手肘與地面平行,前臂自然有依託,手腕不必一直抬高或過度彎折。四十年前,我們用的電腦桌大多配有鍵盤抽屜,把鍵盤和滑鼠放低一些,手肘就自然平行了,很容易達到這個要求。但自從大家改用筆電,鍵盤和滑鼠都跟著筆電攤在桌面上,打字或使用滑鼠時,手腕的位置被抬高、手肘不再能維持平行,時間一長,手指和手腕就一直處在出力狀態,容易出現痠、脹、麻這些狀況。現在也有一些廠商生產所謂的人體工學電腦椅,把兩手平放在扶手上,認為這樣雙手有依託、手臂也能與地面平行——我沒有親自試過,但依他們的圖示,倒也符合我說的原則。只是我覺得,要找到能匹配這種高度的桌子非常不容易;假如桌子能一併解決,那也算是一個方案。不過我仍懷念從前那種便宜的桌子,只要有一個鍵盤抽屜,就能更好地解決這件事。實用的做法:讓鍵盤滑鼠所在的高度落在手肘附近(與手肘同高或略低),同時把螢幕上緣調到約與視線同高——兩件事要一起看,不能只顧手臂、忘了脖子與眼睛。How the desk and chair meet is the most important thing. The ideal is this: using the keyboard or mouse, your elbows are level with the floor, the forearms rest with support, and the wrists don't stay hiked up or bent too far. Forty years ago, most computer desks came with a keyboard drawer — drop the keyboard and mouse a little lower and the elbows came level naturally, easy to achieve. But since laptops took over, the keyboard and mouse sit on the desk with the laptop; while typing or mousing, the wrists ride higher and the elbows can no longer stay level. Over time the fingers and wrist stay under load, and the sore, tight, tingling feelings set in. Some makers now produce so-called ergonomic computer chairs that rest both hands on the armrests, believing this supports the hands and levels the arms with the floor — I haven't tried one myself, but from their diagrams it does follow the principle I describe. The catch, to me, is how hard it is to find a table that matches that height; if the table can be solved alongside it, that's a workable option too. Still, I miss the old cheap desk — with just a keyboard drawer it solved this better. The practical move: keep the keyboard and mouse near elbow height (level with or just below the elbows), and set the top of the screen roughly at eye level. See both together — don't fix the arms and forget the neck and eyes.
  2. 適當休息,別讓手腕一直操勞。工作的時候很容易沉迷,一坐就是兩三個小時不動。建議每一個小時,就安排時間停下來,讓手腕離開鍵盤滑鼠、活動一下手指和手腕。短暫的休息,比週末一次補回來要有用得多。Rest properly — don't let the wrist grind on. It's easy to get lost in work and sit two or three hours without moving. Build a stop into every hour: step away from the keyboard and mouse, and move the fingers and wrist. Short breaks beat one big catch-up on the weekend.
  3. 必要時,配戴護腕來減輕手腕的負擔。情況需要的時候,可以戴上護腕,給手腕一點支撐與提醒,避免一直過度彎折。它不是要您天天戴,而是手腕已經不舒服、或預計要長時間操作時,用來幫自己守住姿勢的一種方式;缺點是戴上護腕後,工作時會有不少不方便的地方,所以也不必長期依賴。When needed, a wrist guard eases the load on the wrist. When the situation calls for it, wear one for a bit of support and a reminder not to over-bend. It isn't meant for every day — it's a way to hold your posture when the wrist is already sore or you know a long session is coming. The downside is that a guard gets in the way of working, so it isn't something to rely on long term either.

三、來訪前,您自己可以先觀察的三件事3. Three things worth noticing before you come

如果願意先留意這幾點,我們會省下不少時間,評估也會更準:If you can notice these before your visit, we'll save time and the assessment will be sharper:

  • 什麼時候最明顯?用滑鼠久了?下午?還是早晨剛起床那一下。When is it worst? After long mouse use? Afternoons? Or that first moment getting out of bed?
  • 哪個角度或姿勢會卡住?避開它時是不是就好些?這能幫忙縮小範圍。Which angle or posture catches? Does it ease when you avoid it? That alone narrows things down.
  • 調整桌椅、休息幾天有沒有比較好?如果幾週都一樣、沒有起伏,才是該認真看待的訊號。Does fixing the setup and resting for a few days help? If it's the same week after week with no ups and downs, that's the signal to take it seriously.

如果調整了姿勢、也安排了休息,還是持續不舒服,來訪時我不會只壓手腕那裡,而是先看您整個上肢的力線和日常使用習慣,找出緊繃真正的來源——會痛的地方,常常只是結果,不是起點。If you've fixed the posture and built in rest but it still won't let go, on a visit I won't just press the wrist. I'll look at your whole upper-limb line and daily habits to find where the tightness truly starts — the sore spot is usually the result, not the start.
