一、肩頸為什麼會緊起來1. Why the neck and shoulders tighten
同樣是「肩頸僵硬」,背後的原因可能很不一樣。把常見的來源攤開來看,會比只盯著那一塊緊的地方有用得多。The same "stiff neck and shoulders" can come from very different places. Laying out the common sources is far more useful than staring at the one tight spot.
- 姿勢與使用習慣。工作桌、椅、螢幕的高度不合身,是上班族最常見的來源。花太多時間低頭看手機、盯著電腦,頸肩長時間處在出力狀態;床墊、枕頭的高度跟身高對不上,睡一晚反而更緊。這些都是每一天都在發生的事。Posture and daily habits. A desk, chair or screen at the wrong height is the most common source for office workers. Too much time looking down at a phone or staring at a screen keeps the neck and shoulders under load for hours; a mattress or pillow that doesn't match your build can leave you tighter after a night's sleep. These are things that happen every single day.
- 運動、車禍或生活意外造成的拉傷。運動傷害、騎車或開車的碰撞、日常裡的跌倒,都可能讓肌肉或肌腱被拉傷。這類來源通常有一個清楚的「從那一天開始」的起點。Strains from sport, collision or accident. A sports injury, a bike or car collision, a fall at home — any of these can strain a muscle or tendon. With this kind of source there's usually a clear "it started the day after that" moment.
- 頸部曾受外力,或隨年紀出現的變化。頸部曾經受過撞擊(例如車禍、跌倒),或者隨著年紀增加而出現的結構性變化,也會讓活動變得不順、容易緊。這類情況評估時要更小心,確認力量該用在哪里、哪些地方不該碰。A past impact, or changes that come with age. A neck that has been hit before (a collision, a fall) or changes that arrive with age can make movement less smooth and the area prone to tightness. These need a more careful look — to confirm where force belongs and where it should not go.
- 精神壓力造成的長期緊繃。長期精神緊繃,會讓肌肉不自覺一直用力;很多人習慣性聳肩而不自知。情緒上的緊,和身體上的緊,往往是同一件事的兩面。Long-term tension from stress. Mental strain keeps muscles clenched without you noticing — many people shrug their shoulders habitually and don't realize it. Tension in the mind and tension in the body are often two sides of the same thing.
- 有些僵硬,來源不在肌肉。不是所有的肩頸僵硬都來自肌肉疲勞。如果僵硬伴隨某些狀況,來源可能不在我們能協助的範圍,要先讓醫師檢查。下面這一段,我想講得精確一點。Some stiffness doesn't start in the muscle. Not every stiff neck comes from tired muscle. If the stiffness comes with certain signs, the source may be outside what we can help with, and a physician should check first. Let me be precise about this.
先做分類,再做處理Sort first, then act
這一點我想特別講,因為它比前面幾項都重要。肩頸緊的時候,第一件該做的事是先分清楚「這一種」和「那一種」,而不是先想怎麼把它弄鬆。I want to give this its own weight, because it matters more than anything above. When the neck and shoulders are tight, the first move is to tell "this kind" from "that kind" — not to reach for a way to loosen it.
這一類,是我們平常在處理的:This is the kind we work with every day:
- 長時間用電腦、看手機之後才出現,起來動一動就鬆些Shows up after long hours on a computer or phone, and eases once you move around
- 忙的時候明顯、放假休息就好些Worse when busy, better after a break or holiday
- 範圍幾個月來差不多,避開某個角度就沒事Roughly the same range for months; fine if you avoid one angle
- 單純的緊、卡,沒有其他的伴隨狀況Just tight or catches, with nothing else going on
這一類,請先讓醫師檢查:This is the kind to check with a physician first:
- 在車禍、跌倒等外傷之後才開始,而且一直沒好轉Began after a collision or fall and simply hasn't improved
- 手臂或手指麻、無力「真的做不到」——手抬不起、東西會掉,而不是只有感覺怪Arm or finger numbness or weakness you genuinely can't work around — an arm that won't lift, objects dropping — not just a weird feeling
- 痛到換任何姿勢都緩解不了,或同時伴隨發燒、體重不明下降Pain that eases in no position at all, or comes with fever or unexplained weight loss
- 頭頸曾受明顯撞擊,之後出現持續的頭暈、噁心或平衡感變差A clear blow to the head or neck, followed by ongoing dizziness, nausea or a change in balance
先做分類,再做處理——順序反過來,才是真正危險的地方。這兩組並不固定:同一個人也可能從左邊走到右邊,所以每次來訪,我們都會重新看一次。Sort first, then act — reverse that order and you're in the dangerous part. The two groups aren't fixed: the same person can move from one to the other, so we look again at every visit.
為什麼我不列出一堆「病名」Why I don't list a string of "conditions"
您可能注意到,這篇通篇沒有出現任何病名。這不是閃躲,而是因為同樣一句「肩頸僵硬」,背後可能是完全不一樣的來源。還沒弄清來源就給一個名字,反而會把人帶偏。我的做法是先把「是哪一股力在拉、哪一處在代償」看清楚,再談接下來怎麼做——這也是為什麼來訪時,我會先花十幾分鐘,而不是一上來就動手。You may have noticed this piece names no condition at all. That isn't evasion — it's because the same words "stiff neck and shoulders" can sit on top of completely different sources. Naming it before finding the source only sends people down the wrong path. My approach is to see clearly which force is pulling and which part is compensating, then talk about what comes next — which is why, on a visit, I spend the first ten-odd minutes looking rather than working.
二、可以自己先做的幾件事2. A few things you can do first
上面那些「需要看醫師」的狀況畢竟是少數。絕大多數的肩頸僵硬,在生活習慣上調整一下,就能明顯舒服一些。以下四件,您在家就能開始。The "see a doctor" cases above are the minority. Most stiff necks ease up noticeably once daily habits shift a little. Here are four you can start at home.
- 選擇高度合身的桌椅。讓長時間工作或讀書時,身體能保持自然、不用額外出力的姿勢。螢幕上緣大約與視線同高,手肘自然垂在身體兩側,肩膀就不必一直聳著。Pick a desk and chair that fit your height. So long hours of work or study don't force your body into extra effort. The top of the screen roughly at eye level, elbows resting naturally at your sides, and the shoulders no longer have to stay hiked up.
- 控制好電腦、手機的節奏。每 40–50 分鐘,起來走動、活動一下。特別是讓眼睛離開螢幕充份休息——頸肩往往也跟著鬆下來,因為您盯著螢幕時,肩頸其實一直沒休息。Pace your screen and phone time. Every 40–50 minutes, stand up and move. Let your eyes rest properly away from the screen — the neck and shoulders often loosen too, because while you stare at a screen, they never really stop working.
- 每天早晚各留 15 分鐘做緩和伸展。針對頸部、上背與肩部,在「有伸展感但不痛」的範圍內慢慢活動,不去追求壓下去的深度。持之以恆,比一次做很久更有用。Set aside 15 minutes, morning and evening, for gentle stretches. For the neck, upper back and shoulders — move slowly within "a stretch but no pain," without chasing depth. Steady practice helps more than one long session.
- 讓心情也放輕鬆。多一點戶外活動,離開整天盯著同一面牆的狹窄環境,對肩頸是一種很實在的釋放。身體的緊,常常是先從心裡鬆下來的。Let your mind loosen too. More time outdoors, away from the narrow space you stare at all day, is a real release for the neck and shoulders. Often the body's tightness lets go only after the mind does.
三、來訪前,您自己可以先觀察的三件事3. Three things worth noticing before you come
如果願意先留意這幾點,我們會省下不少時間,評估也會更準:If you can notice these before your visit, we'll save time and the assessment will be sharper:
- 什麼時候最緊?下午?熬夜後?還是早晨剛起床那一下。When is it worst? Afternoons? After a late night? Or that first moment getting out of bed?
- 哪個角度會卡住?避開它時是不是就好些?這能幫忙縮小範圍。Which angle catches? Does it ease when you avoid that angle? That alone narrows things down.
- 休息一晚有沒有比較好?如果幾週都一樣、沒有起伏,才是該認真看待的訊號。Does a night's rest help? If it's the same week after week with no ups and downs, that's the signal to take it seriously.
不用整理得很完整,想到什麼講什麼就好。這些沒有標準答案,但每一個回答,都能讓「找位置」這件事快一點、準一點。No need to prepare anything formal — just say what comes to mind. There are no right answers, but every answer makes finding the source a little faster and a little more accurate.
