一、為什麼搬重物最容易閃到腰1. Why lifting is where backs get hurt

同樣是搬東西,有人搬了一輩子沒事,有人一次就站不起來。差別往往不在重量,而在「身體怎麼擺、力從哪裡出」。絕大多數腰部在搬重物時的不適,都和姿勢、動作有關;真正的外傷(例如跌倒、碰撞)另當別論。Two people, same load — one lifts for a lifetime without trouble, the other can't stand up after one try. The difference is rarely the weight; it's how the body is placed and where the force comes from. Most lower-back trouble during lifting traces back to posture and movement; a genuine impact (a fall, a collision) is a different matter.

  1. 彎腰就搬,是最省事也最傷腰的動作。把東西從地上拉起來時如果直著腿、彎著腰,重量幾乎全壓在腰部,腿完全沒幫上忙。這是最方便的動作,卻也是後遺症最多的——當下沒感覺,隔天往往才來。Bending at the waist is the easiest move and the hardest on the back. If you lift something off the floor with straight legs and a rounded back, the weight lands almost entirely on the lower back while the legs do nothing. It's the most convenient move, and the one with the most aftermath — often you feel fine, then it shows up the next day.
  2. 搬起來之後又扭轉身體。東西已經在手上,身體一轉去放——這一瞬間,腰部被強行扭轉,形成額外的剪力,肌肉與關節承受了原本不該有的壓力。很多「閃到腰」,其實是這一下扭出來的。Then turning the body while holding it. With the load already in your hands, you twist to set it down — in that moment the lower back is forced to rotate, creating extra shear, and the muscles and joints take pressure they were never meant to. A lot of "thrown backs" come from exactly this twist.

先做分類,再做處理Sort first, then act

這一篇主要談「怎麼搬才不傷」,是預防。但如果您已經不舒服了,第一件事不是急著處理,而是先分清楚「這一種」和「那一種」This piece is mostly about lifting without getting hurt — prevention. But if you're already sore, the first move isn't to rush at it; it's to tell "this kind" from "that kind."

這一類,是我們平常在處理的:This is the kind we work with every day:

  • 搬完隔天慢慢痠脹,休息幾天就明顯好轉Aches the next day, clearly better after a few days of rest
  • 範圍局限在腰部肌肉,避開某個角度就還好Stays in the lower-back muscles; fine if you avoid one angle
  • 沒有麻、沒有使不上力的地方No numbness, nothing you genuinely can't do

這一類,請先讓醫師檢查:This is the kind to check with a physician first:

  • 在跌倒、車禍等外傷之後才開始,而且一直沒好轉Began after a fall or collision and simply hasn't improved
  • 腿或腳麻、無力「真的做不到」——腳墊不起、東西會掉,而不是只有感覺怪Leg or foot numbness or weakness you genuinely can't work around — a foot that won't lift, objects dropping — not just a weird feeling
  • 痛到換任何姿勢都緩解不了,或同時伴隨發燒、體重不明下降Pain that eases in no position at all, or comes with fever or unexplained weight loss

先做分類,再做處理——順序反過來,才是真正危險的地方。這兩組並不固定:同一個人也可能從左邊走到右邊,所以每次來訪,我們都會重新看一次。Sort first, then act — reverse that order and you're in the dangerous part. The two groups aren't fixed: the same person can move from one to the other, so we look again at every visit.

為什麼我總說「先護腰,再出力」Why I keep saying "protect the back before you push"

搬東西閃到腰,痛的位置在腰,但源頭常常不在腰——而是腿沒出力、身體位置錯了,腰才被迫去扛它不該扛的。這正好呼應我常說的一件事:會痛的地方,不一定是問題開始的地方。所以我寧可慢一點、多走兩步、蹲下去用腿力,也不願為了省那一下,把風險全壓在腰上。這也是為什麼來訪時,我會先花十幾分鐘把來源看清楚,而不是一上來就動手。When a lift hurts the back, the pain is in the back — but the source often isn't. The legs didn't engage, the body was in the wrong place, and the back was forced to carry what it shouldn't. That echoes something I say often: where it hurts is not always where it began. So I'd rather go slower, take two extra steps, squat and use the legs, than dump all the risk onto the back to save one motion. It's also why, on a visit, I spend the first ten-odd minutes finding the source before I do anything.

二、正確的搬法,四個要領2. The right way to lift — four points

觀念其實很簡單:讓粗壯的腿來扛重量,讓腰只負責「穩住」,而不是「硬拉」。四個要領,馬上能用:The idea is simple: let the strong legs carry the weight, and let the back only "hold steady" rather than "yank." Four points you can use right now:

  1. 靠近再搬,別隔空拉。盡量貼近被搬的物品再下手。物件離身體越遠,力臂越長;力臂越長,施加在腰部的壓力就越大。先上前一步,比事後養傷便宜得多。Get close before you lift. Move up to the object rather than reaching for it. The farther the load sits from your body, the longer the lever — and the longer the lever, the more pressure lands on the lower back. One step closer beats weeks of recovery.
  2. 用大腿,不要偷懶用腰。採蹲姿:臀部往後、膝蓋彎曲,靠腿部粗壯的肌肉把身體和重物一起撐起。腰椎不該是第一個出力的人——把力氣花在腿上,腰才保得住。Use the thighs, not the lazy back. Squat: hips back, knees bent, and let the big leg muscles lift both you and the load. The lumbar spine should never be the first to engage — spend the effort in the legs and the back stays safe.
  3. 要轉向,用腳走,不扭腰。重物上身後,若要改變方向,靠腳步移動整個身體去對準,而不是扭轉腰椎。多走兩步路,腰就少受一次罪。To change direction, walk — don't twist. Once the load is up, if you need to face another way, move your feet and turn the whole body; don't rotate the lumbar spine. A couple of extra steps spares the back another hit.
  4. 善用工具,才是聰明人。手推車、分裝的背包、底部的滑墊——能把「搬」變成「移」,就別硬扛。省力不是偷懶,是把風險實實在在降下來。Use tools — that's the smart move. A cart, a split backpack, a slider under the load — if you can turn "lifting" into "moving," don't brute-force it. Saving effort isn't laziness; it's lowering the risk for real.

三、已經不舒服了,先別硬扛3. Already sore? Don't just push through

如果讀到這裡,您想到的是某次搬完之後的腰,先留意這三件事,我們評估時會更準:If reading this brings back a certain lifting episode, notice these three things — they'll make our assessment sharper:

  • 是慢慢來,還是當下就動不了?隔天慢慢痠,和當場「啪」一聲站不起來,是兩回事。Did it build up, or stop you on the spot? Aches the next day and a back that won't move that same moment are not the same thing.
  • 休息一兩天有沒有好轉?幾週都一樣、沒有起伏,才是該認真看待的訊號。Does a day or two of rest help? If it's the same week after week with no change, that's the signal to take it seriously.
  • 有沒有麻、無力,或某條腿使不上力?只要有一項,先去看醫師,不用猶豫。Any numbness, weakness, or a leg that won't cooperate? If so, see a physician first — no need to hesitate.

不用整理得很完整,想到什麼講什麼就好。每一個回答,都能讓「找位置」這件事快一點、準一點。No need to prepare anything formal — just say what comes to mind. Every answer makes finding the source a little faster and a little more accurate.
